Enrollment and Withdrawal:

 

1.    Children must meet the age requirements as outlined by the Archdiocese. 

·    Pre-kindergarten student to be 3 and 4 years old on or before September 1st.

·    Kindergarten student to be 5 years old on or before September 1st.

·    First grade student must be 6 years old on or before September 1st.

·    An original birth certificate (not a hospital copy), social security card and baptismal certificate (if applicable) must be presented at time of enrollment.

 

2.    All Pre-kindergarten students must be completely potty trained.

 

3.    A completed health record signed by a doctor is required at time of registration. Immunizations must be up to date.

 

 

Immunization Requirements:

 

 

3 and 4 year olds:

·     Polio – 3 doses

·     DTP/DTaP – 4 doses

·     MMR – 1 dose on or after 1st birthday

·     HIB – 1 dose on or after 15 months unless a primary series and booster was completed prior to, or at, 15 months of age.

·     Hepatitis A – 2 doses

·     PCV7 - 2 doses

  *It is recommended that DPT/Polio and 2nd MMR be given as soon as possible after the 4th birthday. They are required by the 5th birthday.  Varicella – 1 dose if born after September 2, 1994 .

 

5 and 6 year olds:

·     Polio – 3 doses with one on or after the 4th birthday.

·     DTP/DTaP/TD – 4 doses with one on or after the 4th birthday.

·     MMR – 2 doses – first given on or after the 1st birthday and second given no sooner than 28 days from the first doses.

·     HEPITITIS B – 3 doses

·     Varicella – 1 dose

 

7 Year olds:

·     Polio – 3 doses with one on or after the 4th birthday.

·     DTP/DTaP/TD – 3 doses any combination with one received on or after the fourth birthday.

·     MMR – 2 doses – first dose given on or after the first birthday and second dose no sooner than 28 days from the first dose.

·     HEPITITIS B – 3 doses if born after September 2, 1992 .  It is recommended for all students.

·     Varicella – 1 dose if born after September 2, 1994 .


 

Middle School requirements are:

·      DTP/DTaP – 3 doses (any combination) with one dose on or after the fourth birthday AND within the last ten years.

·      Polio – 3 doses with one dose after the fourth birthday.

·      MMR – 1 dose on or after the first birthday and a second dose by the 12th birthday if born between September 1, 1978 , and before September 1, 1991 .

·      Hepatitis B – 3 doses.

·      Varicella – 1 dose if born before September 2, 1994 .

 

Middle School students who have immunizations due during the fall semester will be required to have all immunizations before the beginning of the fall semester, and students who have immunizations due in the spring semester will be required to have all immunizations before the beginning of the spring semester.  Students already enrolled in SMMCS will be notified in May for the fall semester and October for the spring semester.

 

4.   New families must submit their applications with a $100.00 application fee (one time fee per family) and $450 student fee at time of registration.  These fees are non-refundable.

 

5.   New students registering must provide copies of the last report card and latest standardized test scores.  All new students for kindergarten through seventh grade will be tested before placement unless current standardized test scores are available. 

 

6.   Students do not have to be Catholic to enroll; however, priority for admission will be given to applicants who are members of St. Mary Magdalene Parish or have siblings already in St. Mary Magdalene School. Catholic students from other parishes will be considered next and finally, open to all other students. Vacancies will be filled from a waiting list. Admission will be based on the date of their parent’s registration in the parish, which is verified at the Parish Office. Other students will be accepted in the order of registration and the completeness of their applications.

 

7.   All students entering and returning to St. Mary Magdalene Catholic School are on an academic and behavioral probation during each school year.  Acceptance of transfer students will be based on the most recent standardized test scores, report cards, teacher recommendations and placement exam results.
